### Notes and assistant context

* Added source references to generated clinical notes so supporting information can be inspected from the note.
* Made the patient and encounter information attached to assistant conversations visible in the chat input.

### Fixes

* Fixed task comment creation and missing encounter notes in the encounter view.
* Fixed note generation when an encounter has an empty transcript.
