Notes and assistant context
- Added source references to generated clinical notes so supporting information can be inspected from the note.
- Made the patient and encounter information attached to assistant conversations visible in the chat input.
Fixes
- Fixed task comment creation and missing encounter notes in the encounter view.
- Fixed note generation when an encounter has an empty transcript.
